This page is intended for students interested in receiving
disability accommodations at Mount Saint Mary College.
Please follow all three steps described below in order to
receive accommodations in a timely manner. If you
have any questions, please contact us by telephone at 845-569-3115,
or by email at disabilityservices@msmc.edu
1. Information and Accommodations Request Form
Complete and submit this online request form, which will open in
a new window: Information and Accommodations Request
2. Verification Forms
Print out the appropriate verification forms/instructions and
have a qualified professional (as described on each form) complete
them.
These forms provide formats and/or instructions for
diagnosticians (e.g., psychologists, physicians, etc.) to present
our office with appropriate documentation of a student’s
disability(s). It is important to complete the forms as instructed
in order to receive accommodations in a timely manner.
Please note that some students may need to have more than one
form completed depending on the disability(s) and type of
accommodation(s) requested.
Form A - Instructions for qualified professionals to
submit appropriate documentation for students with Learning
Disabilities.
Form
B - To be completed by a qualified professional for a
student with a Psychological Disorder and/or Attention
Deficit Disorder.
Form
C - To be completed by a qualified professional for
students requesting accommodations for a Physical and/or
Sensory Disability.

the Mount Saint Mary College campus.
